UNM Center for Inclusive Health

The UNM School of Medicine Department of Internal Medicine’s Center for Inclusive Health launched the Mobile Medical Team, known as Street Medicine, to break down barriers to quality health care. We bring acute, chronic, and preventive services right to where patients live: on the streets, in shelters, or in temporary housing sites.

We’re an interprofessional, interdisciplinary group committed to serving our community’s most vulnerable:

 • Physicians specializing in internal medicine
 • Nurses skilled in complex care coordination
 • Ambulatory Practice Professionals (Physician Assistants & Nurse Practitioners)
 • Behavioral Health Clinicians addressing mental health needs

The Mobile Medical Team “meets patients where they are,” delivering high-quality care in nontraditional settings as encounters occur on the street, in shelters, or within temporary housing sites:

 • Acute medical evaluation and treatment
 • Chronic disease management
 • Preventive medical services
 • Mental health care

By meeting patients where they are and embedding care directly into communities, Street Medicine aims to:

 • Improve overall health outcomes
 • Reduce unnecessary emergency department visits
 • Lower inpatient hospital admissions
 • Decrease crisis intervention contacts

In the Fall of 2022, a generous donation brought the Street Medicine Institute to UNM Health to build a solid foundation to develop best practices for patient-centered care. Since that time, private donations have funded vehicles for the team and launched our new mobile pharmacy.

Your gift helps the Center for Inclusive Health and the Street Medicine Team lead our community in addressing the vast social and medical disparities affecting the unsheltered in Albuquerque and across New Mexico.
